Service Provisions – Living Care Specialist

Living Care Specialist is an NDIS-registered service provider delivering plan management, support coordination, and participant assistance services to individuals under the National Disability Insurance Scheme (NDIS).

This Service Agreement is made between Living Care Specialist and the Participant or Nominated Representative. It takes effect from the date signed and continues for the duration of the participant’s engagement with Living Care Specialist, unless otherwise advised in writing.

Our Responsibilities
  • Living Care Specialist will:
  • Provide NDIS-funded services in line with NDIS guidelines and your approved plan.
  • Offer plan management and financial intermediary support as required.
  • Reconcile balances and ensure accurate budget tracking.
  • Pay supplier invoices promptly on behalf of participants.
  • Process reimbursements efficiently and transparently.
  • Track spending against your approved budgets.
  • Provide statements and reports upon request.
  • Support purchases and provider coordination where appropriate.
  • Resolve issues quickly and communicate updates clearly.
  • Treat all participants and representatives with courtesy and respect.
  • Listen to feedback and act to improve services.
  • Protect your privacy in accordance with our Privacy Policy.
Participant / Representative Responsibilities

You agree to:

  • Provide all necessary information to Living Care Specialist in a timely manner.
  • Treat our staff with courtesy and respect.
  • Contact us directly with any concerns or feedback.
  • Notify us immediately if your NDIS plan changes, is suspended, or you cease to be an NDIS participant.

Plan Management Process
By nominating Living Care Specialist as your NDIS service provider, you authorise us to access your NDIS funding through the NDIS online portal (PRODA) to:
  • Set up your services and manage your plan.
  • Process and pay invoices to providers.
  • Handle reimbursements and generate reports.
By signing this agreement, you consent to Living Care Specialist transferring funds within your Core Budget between categories when needed and administering invoices and reimbursements accordingly.

Either party may end this agreement with one month’s written notice. In cases of serious breach, the notice period may be waived.
